What is All-on-4?
All-on-4 is a full-arch restoration concept: instead of replacing every tooth with its own implant, an entire arch of teeth is carried by four implants. The two rear implants are placed at an angle, which lets them anchor in denser bone toward the front of the jaw and often avoids the need for bone grafting or sinus lifts.
The result is a bridge that is fixed in your mouth, you do not take it out at night, it does not move when you eat, and it does not cover your palate. For patients wearing a loose full denture, or facing the loss of a failing dentition, it is usually the most life-changing treatment in dentistry.
Temporary vs final prosthesis, the honest version
All-on-4 is a staged treatment. At the first visit, implants are placed and, where stability allows, a temporary fixed bridge is attached so you leave with teeth. That temporary is deliberately lighter, it protects the implants while bone integrates. After healing, at a second visit, the definitive prosthesis is crafted and fitted: stronger materials, refined aesthetics, adjusted bite. Clinics that blur the line between the day-one temporary and the final bridge are setting expectations you will collide with later. We would rather explain it clearly now.
If the implants are not stable enough on the day for an immediate bridge, the safe protocol is a removable temporary while bone heals. The dentist decides this on measured stability, not on marketing.

Risks, honestly
All-on-4 concentrates a full arch onto four implants, so each implant matters. Individual implant failure rates are low, and the protocol includes contingencies, but smoking, uncontrolled diabetes and poor hygiene raise risk here just as with single implants. Post-surgical infection, temporary numbness and prosthesis wear are the other realities worth naming. What protects you is planning on good imaging, honest stability measurement on the day, and your own maintenance discipline afterwards.

Do I really get teeth on the same day?
Usually, yes, a fixed temporary bridge is attached once implant stability is confirmed. If stability is insufficient, the safe route is a removable temporary during healing. You will know which applies to you on surgery day, and why.
How is the temporary different from the final bridge?
The temporary is lighter, designed to protect healing implants, and aesthetically simpler. The final prosthesis, made months later, uses definitive materials such as hybrid zirconium, with refined shade, shape and bite. Both stages are itemised in your quote.
How many trips to Tirana do I need?
Two for the full treatment: surgery plus temporary bridge on the first, final prosthesis on the second, typically 3–6 months later. Each visit is usually under a week.
Is All-on-4 better than All-on-6?
Neither is universally better. Four angled implants shine where bone is limited; six implants spread load further where bone allows. Your X-ray decides, and we explain the reasoning either way.
What happens to my remaining teeth?
If the arch is being fully restored, non-saveable teeth are removed during surgery. If several of your teeth are genuinely saveable, we will tell you, a full-arch is not automatically the right plan.
Can I eat normally afterwards?
With the temporary bridge, a soft-to-moderate diet protects healing. Once the final prosthesis is fitted, most patients eat essentially without restriction, with more chewing power than any denture.
How do I clean a fixed full arch?
Under-bridge cleaning with interdental brushes or a water flosser, daily, plus professional maintenance visits. We demonstrate the routine before you fly home and put it in writing.
